Transitioning into a healthier YOU just got easier!
The founder of Transitional Wellness Center. I am dedicated to the health and wellness of each and every person who walks through the door.
After serving 25 years in the military as a nurse corps officer (Lt Col Retired) and experiencing my own personal health crisis, I knew it was time to make some life-changing decisions.
Having recognized that my own health issues were not changing like I was told they would. I decided to use my knowledge of medicine to find other interventions that would help me create a holistic, optimal, and healthy lifestyle.
So, I started rebuilding and restoring my own health through nutrient applications and functional/integrative medicine approaches. While on my journey I was introduced to the American Academy of Anti-Aging Medicine (A4M). I was dedicated to making sure there was a way to improve my health and self-image without making drastic, expensive changes to my diet and activity.

Functional Medicine looks at the root cause of chronic disease as opposed to only treating the symptoms you're experiencing. It also recognizes that the patient’s environment may play a pivotal role in their health. That environment can include their nutrition, exercise, stress, relaxation, and exposure to toxins.
Functional Medicine is very personalized and you are involved in each step toward your recovery. Functional Medicine believes that many chronic diseases are preventable and even reversible. To do that one should adhere to their individualized plan addressing points such as diet, stress, exercise, genetics, and psychospiritual needs with their Functional Medicine coach.
Nurse Practitioner, Tara Davis's goal is to provide medical consulting/health education and coaching services from a more comprehensive Functional Medicine perspective. It is not our intention to be in competition with other medical providers but to complement previous and ongoing care. We will capitalize on our 13 years of experience in primary care family medicine as we use the science-based Functional Medicine approach to more comprehensive evaluations of one’s health and disease risk. Our Functional Medicine philosophy towards health and medical care is that the whole person must be addressed if we are to maximize the goal of excellent outcomes. This means that appropriate attention must be given to all of the primary components of our health and wellness, i.e. spiritual health, physical health, mental-emotional health, and relational health. When there is disease or dysfunction in one of these components, there will be negative consequences affecting the other areas. When one is experiencing wellness and good health in one or more of the components, the reverse is true.
This Functional Medicine office is a place of hope, healing, and wholeness regardless of race, gender, spiritual orientation, and lifestyle. We acknowledge that healing and wholeness are God’s will. We adhere to the belief that all healing, even miracles, are from God and are accessible through the integration of excellent medical care with His word, prayer, and the present-day workings of the Spirit of God.
The Functional Health Report uniquely organizes and creates an interpretation providing a comprehensive insight and assessment into the state of previously hidden health trends of the main body systems, along with the reporting on the nutrient and trend to and from clinical dysfunction.
We're committed to walking with you as you make difficult lifestyle and diet changes by providing you with professional medical help along the way.
Evidence-based nutritional supplements are available for purchase at the office and in our online store.
